Files and folders with %[environment variables]% are not properly processed, the environment variable is falsely expanded.
Assume a folder with name "%HOMEPATH%" is located in "B:\Downloads".
When you search for "%HOME" and double-click it, the error occurs.
What's more, you cannot use "%HOMEPATH%" as the search term to locate that folder, since the search term is also falsely expanded in this case. And all files and folders in that folder cannot be accessed via Everything.
The same applies to files.
I know that in most cases, it's convenient to expand environment variables, and that's a bad habbit to have names such as "%HOMEPATH%". But there are some such folders in my PC. So sad.

Sometimes even Windows can throw such errors , it it is unable to parse the file name. Earlier happened to me too.. Seems like Windows error..
